The Failure Mode Effects Analysis (FMEA) is a versatile tool that has been in use for decades and has been adapted to most industries. An FMEA can be used on component / systems / subsystems designs (DFMEA) or the processes (PFMEA) used to manufacture components / systems / subsystems. From small agricultural enterprises with concerns about personnel safety or execution efficiency to complex space launch vehicle design the FMEA methodology can be applied to evaluate and guide risk mitigation. As product designers, we need to understand the potential risks our products expose customers to and it is our responsibility to quantify and mitigate unacceptably high risks before a product launches.
